We are looking for enthusiastic and compassionate Field Registered Nurse Assessors to join our dedicated team at Maximus. In this vital role, you will provide essential home assessment services to some of New York’s most vulnerable populations as part of the New York Independent Assessor Program (NYIAP).

Your Mission:

As a Field Nurse Assessor, you will take on the rewarding responsibility of conducting comprehensive assessments to evaluate eligibility for community-based long-term services. Utilizing the Community Health Assessment tool, you will perform conflict-free assessments of individuals, helping them access the care they need. This role grants you the opportunity to collaborate with our clinical leadership team, conducting assessments primarily in the Bronx, specifically in the University Heights, Morrisania, and Yankee Stadium areas.

What You’ll Do:

– Travel Responsibly: Conduct clustered field visits, focusing on specific neighborhoods within the Bronx.
– Comprehensive Assessments: Execute in-home, in-facility, or telehealth assessments to evaluate individuals’ health status, strengths, care needs, and preferences using the UAS-NY assessment tool.
– Cultivate Relationships: Build and maintain positive rapport with consumers, caregivers, and facility staff to foster a supportive environment.
– Ensure Accuracy: Diligently review medical documentation, reporting discrepancies or concerns encountered during evaluations.

Your Responsibilities Include:

– Reviewing determinations for compliance with regulations.
– Making medical necessity determinations for assigned cases.
– Resolving technical issues during the reconsideration process.
– Conducting case reviews to identify and summarize facts and issues.
– Engaging in special projects, including legal and medical research when necessary.
– Documenting assessment data accurately in the UAS-NY online application and ensuring proper coding per guidelines.
– Maintaining up-to-date knowledge of community resources and the UAS-NY assessment process.
– Addressing any consumer concerns or conflicts identified during evaluations, as needed.

Qualifications We’re Looking For:

– An active RN license and a minimum of two years of clinical experience.
– At least two years of experience in home care.
– A valid RN license in New York State and local travel capability.
– A reliable mode of transportation (ride-sharing options accepted).

Preferred Experience:

– Familiarity with the UAS-NY Community Assessment tool.

Home Office Setup:

– Company-provided cellphone and computer equipment.
– High-speed internet service with a minimum of 100 Mbps download speed.
– A secure and private workspace suitable for remote work.

Compensation:
